The following documents and information are required at the time of registration:
Certified Birth Certificate
Update Shot Record
School Physical Form (Must be less than one year old when school begins & also must include Hematocrit or Hemoglobin
Verification of Dental Exam (Within a year of school start date)
Proof of Florida Residency: Two documents containing name and street address of parent (No Post Office Box) (4 year old children only) (Ex.: drivers license, utility bill, lease agreement, etc.)
